The theme of the add-on is based on the events that accompany the return to Windurst of the eccentric doctor Shantotto. Friends who remember her from before her sudden and unexpected disappearance are surprised by her unusual behaviour. At one point, Shantotto proclaims herself the ruler of her new empire. The player’s task is to discover what is behind this change of personality.

This mini-player is a series of quests accompanied by film interludes. The player stands up against two bosses and the reward, as in previous scenarios, are valuable items. Patch uses already present in the game elements of the environment, terrain and monsters but enriches it with a new feature line.

Shantotto Ascension requires the installed Rise of Zilart plugin, which was sold in the United States and Europe (and thus in Poland) in the basic version of the game. New tasks are available for characters at level 10 or above.
